We complete a range of assessments with service users who have complex needs. This assessment may take place in an individual’s own home, in a temporary situation (for example, in hospital), or in a care environment (for example, residential care). Assessments like these focus on supporting people to make choices about where they want to live, and how they want their care needs to be met. It may also include assessing an individual’s capacity to make decisions, and making the decision in their best interests if they lack the capacity. At the end of the process, we aim to have a clearer idea about the options available to the service user, if any extra funding is needed, and how to coordinate and review care arrangements.
