Older People

We support older people over the age of 65, as well as people with a physical disability. The service is structured into Complex Care Teams, which usually consist of a number of Social Workers, Occupational Therapists and Community Care workers (employed by Devon County Council). These professionals work alongside the Community Matron to support people with long term conditions who are living in their own homes in the community.

The Complex Care Teams enjoy strong relationships throughout the voluntary and community sector. They are also able to draw upon a comprehensive range of re-ablement resources. These resources ensure service users can make real choices about where they live and how support is delivered to them. This includes, access to an Enabling Service which helps people resume hobbies and social group membership.

If you join the Adult and Community Services Complex Care Teams Council, you will benefit from a challenging, supportive working environment and a comprehensive range of resources. We will provide clear guidance regarding the quality of work expected, and you will have all the supervision and support you need. You will also be supported day-to-day by your frontline Practice Manager and become part of a professional (Social Work) support network.
